If you are wanting to use your out-of-network benefits with your insurance, please let me know and I will provide you with a “superbill.” This is an itemized receipt that includes all of the necessary information needed to submit for reimbursement of services.

When using out-of-network benefits you will pay for the services provided up front and submit for reimbursement from your insurance company. I encourage you to reach out to your insurance company to get more information on the specific benefits with your plan.

We can also discuss using a sliding scale arrangement in combination with your out-of-network benefits.

It is important to me to offer accessible services. If my full fee is not doable for you right now, we will discuss what a good fee is for you at the moment.

Once we have agreed on a fee, I will send you a Reduced Rate Fee Agreement Form that will state the reduced fee, as well as the date we will reevaluate our fee arrangement.

Reduced rate fees can also be used in combination with your out-of-network benefits.

Payment is accepted using a credit or debit card, including Health Savings Account (HSA) or Flexible Spending Account (FSA) cards. A credit/debit card will be required to be on file and will be charged at the end of each session.

If you cancel within 48 hours of your scheduled session, you will be charged the full amount of your session with the payment method on file.

A Good Faith Estimate is a statement informing you of the cost of services.

Federal law requires that health care providers inform clients/patients of the cost of services provided if they do not have or are not using insurance for the services.

You will receive a Good Faith Estimate detailing the cost of your services with me in your new client paperwork. This document may be updated after the initial intake and assessment process has been completed.
